New Patients

To register with our practice please complete the form in the surgery, please ask our receptionist. Alternatively, you can find a PDF version below. We do not discriminate on the grounds of race, gender, social class, age or religion, or of disability or medical condition. Our only stipulation is that you live within the practice area.


Although under the “new contract” you are registered with the practice, rather than an individual GP, you have a right to express a preference to see an individual doctor. Indeed we would encourage you to see the same doctor (of your choice) for most routine visits to the surgery as this helps to ensure continuity of care. Please tell any member of the team if have such a preference and we will record it in you file.

After registration we invite you to a new patient medical at which we can check for any ongoing medical problems and explain the workings of the practice. Please bring a sample of urine to this appointment.

Medical treatment is available from the date of registration. Please contact reception for further information.
